This letter is to notify you that the Schools and Libraries Division (SLD) of the Universal Service Administrative Company (USAC) has received and accepted an FCC Form 486, Receipt of Service Confirmation Form, from you. This notification is to confirm the information that you provided. This information is being shared with the service provider whose SPIN you identified on each Funding Request Number (FRN).

ERC Scope Of Work

1. Record date of receipt.

2. Check for accuracy

3. Scan for document warehousing.

4. Discuss with client about billing preference.

5. Notify vendors of billing preference.

6. Send receipt notification to client with pdf attachment of 486 Letter.

Retention of Records and Audits

• Applicants MUST maintain their records for at least five years to be able to comply with audits and other inquiries or investigations. USAC and the FCC visit a sample of applicants to ensure services have been delivered in compliance with FCC rules. Once a client, ERC maintains these records on your behalf in both a paper hard copy and for easy access, a secure online accessible data warehouse.
