The Form 470 signals to the service
provider community and to the SLD that an applicant is interested in
procuring certain E-rate eligible products, or services, or both.
Form 470 can be filed for a particular funding year as soon as the
Schools and Libraries Division (SLD) posts a notice to that effect
on this web site. This notice will be posted at least 12 months
before the start of that funding year.
Form 470 must be POSTED on USAC's web
site for at least 28 days before an applicant can execute a contract
for contracted services, select a service provider for tariffed or
month-to-month services, and sign and submit a Form 471.
Purpose of the Form
The Form 470 cannot be completed by a
service provider who will participate in the competitive process as
a bidder. If a service provider is involved in preparing the Form
470 and that service provider appears on the associated Form 471,
this will taint the competitive process and lead to denial of
funding requests that rely on that Form 470.
ERC Scope of
Work

Identify, analyze, and consult
with entity regarding new contract(s), contract renewal(s),
and/or master contract issues.

Prepare Form 470(s) for on-line
filing.

Consult with entity regarding
on-line certification.

Deliver Form 470 certification
page(s) to an authorized employee of the entity for signature.
Retention of Records and Audits

Applicants MUST maintain their
records for at least five years to be able to comply with audits
and other inquiries or investigations. USAC and the FCC visit a
sample of applicants to ensure services have been delivered in
compliance with FCC rules. Once a client, ERC maintains these
records on your behalf in both a paper hard copy and for easy
access, a secure online accessible data warehouse.
