This letter is to notify your
Service Provider that the Schools and Libraries Division (SLD)
of the Universal Service Administrative Company (USAC) has
received and accepted a Form 472 from you their client.
The SLD commits to reimburse
the service provider the discounted portion of the cost of
eligible services provided to you their client based upon
one or more Forms 471.

Send receipt notification to
client and their Service Provider an email with pdf attachment
of 486 Letter.
Retention of Records and Audits

Applicants MUST maintain their
records for at least five years to be able to comply with audits
and other inquiries or investigations. USAC and the FCC visit a
sample of applicants to ensure services have been delivered in
compliance with FCC rules. Once a client, ERC maintains these
records on your behalf in both a paper hard copy and for easy
access, a secure online accessible data warehouse.
